Information for Participating Veterinarians

Application and Payment Procedures for the Spay/Neuter Program

HART'S Spay/Neuter Program No Longer Includes Pawsitive Prevention

Previously issued Pawsitive Prevention Certificates will be honored for Spay/Neuter Only

  • Do not charge vaccinations or any other services to HART
  • HART does not pay for any additional surgeries or complications from surgery
  • Clients CAN receive applications from HART or a participating veterinarian
  • Clients should mail a completed application to HART with a check or money order
  • If qualified, the client receives a Spay/Neuter Certificate from HART by mail
  • Clients should make their own appointment with a participating Veterinarian
  • Client brings the animal and Certificate to the clinic at the scheduled time
  • The surgery is performed. The Certificate is completed and signed by the Veterinarian
  • Do not add sales tax. HART is a tax exempt organization.
  • All payments under this program are made through our credit card account
  • Please do not charge HART for services provided to spay/neuter programs operated by the Chattanooga Humane Society, Tennessee Humane Animal League, CARE, etc.
  • Attach the credit card receipt to the LOWER LEFT FRONT CORNER of the Certificate
  • Fax the certificate(s) to: (423) 875-4911
  • Retain the original receipt and the certificate in the your files
  • If you do not have a fax, you may mail the original receipt and Certificate to us at HART, P.O. Box 97, Lupton City, TN 37351
  • Process each Certificate Separately making sure the date, Certificate number, and amount are bold enough to be read on the fax received by HART
  • Process each Certificate in a timely manner as we do a weekly accounting
  • The credit card receipts and the related Certificates are the only records we need--We do not need the invoices or statements printed from your accounting system.
  • Please make every effort to protect the account number and restrict access to those employees not directly involved and knowledgeable with the HART payment process
